Acorn and Oak Research Proposal and Application Checklist

Use this checklist to ensure all required information and documents are included in your grant application submission.

Acorn and Oak Proposal Template

Follow the proposal template to format your submission. Acorn proposals are limited to 8 pages and Oak proposals limited to 12 pages.

Pre-Proposal Template

This pre-proposal template is part of a two-stage application process and outlines the required sections investigators must complete to summarize their project aims, relevance to canine health, and research approach for initial review.

Lay Summary Template

The lay summary provides a concise and accessible explanation of the research proposal, highlighting the project’s approach and impact.

Biosketch Template

The biosketch briefly highlights the applicant’s qualifications, experience, and contributions to the field, within a 2 page limit.

Budget Template (Excel)

The Excel budget template details costs for personnel, supplies, and other expenses, with indirect costs limited to 15% of the total grant budget.

Budget Template (Word)

The Word budget template details costs for personnel, supplies, and other expenses, with indirect costs limited to 15% of the total grant budget.

Industry Involvement Disclosure

Applicants are asked to indicate whether they consent to corporate representatives reviewing their grant and if they are open to direct contact with corporate reviewers during the process.

Signature Page Template

The signature page includes the authorized signatures of the investigator and institutional officials, confirming accuracy and approval of the application.

STEP 2

Grant Awardee Templates

Once your grant is awarded, use these templates to manage your project’s progress, track milestones, and report key findings according to grant requirements.

Interim Reporting

Submit interim reports every 6 months through the online grantee portal to update the Foundation on your project’s progress, outcomes, and impact.

Acorn and Oak Grant Progress Report

These reports are required to assess study progress and ensure the work is meeting its objectives. Non-confidential information may be shared with relevant donors.

Canine Sports Medicine and Rehabilitation Residency Progress Report

These reports track the development of the trainee and effectiveness of the training program. Non-confidential information may be shared with relevant donors.

Small Animal Theriogenology Residency Progress Report

These reports track the development of the trainee and effectiveness of the training program. Non-confidential information may be shared with relevant donors.

Extension and Amendment Requests

Request project extensions or amendments using the appropriate forms below. For requests not available in the online investigator portal, please email your request to chfgrants@akcchf.org.

No-Cost Extension Request

A no-cost extension allows you to extend the project timeline without additional funding. For no-cost extension requests that include a reallocation of funds, please also submit a budget reallocation request.

Protocol Amendment

A protocol amendment requests a change to the approved project scope or methodology. If your amendment request includes a reallocation of funds, please also submit a budget reallocation request.

Investigator Amendment Request

An investigator amendment is required when there is a change in the Principal Investigator (PI) or Co-Investigator (Co-I). The new PI or Co-I must also authorize the Humane Use of Animals policy.

Adverse Event Report

An adverse or serious adverse event report must be completed as per the grant agreement and submitted for timely review.

Mentor Amendment Request

A mentor amendment is required when there is a change in the designated mentor for the training program. The new mentor must be approved by the Foundation.

Humane Use of Animals Policy

All investigators must sign the Humane Use of Animals policy to ensure regulatory compliance and ethical treatment of animals in research. This signed agreement is required for all projects.

Budget Reallocation Request

A budget reallocation request must be submitted to move funds within an approved budget due to changes in project needs or expenses and ensure consistency with the approved plan.

Budget Adjustment Request

A budget adjustment request must be submitted to amend the approved budget due to unforeseen changes in project scope or expense, ensuring the revised budget aligns with project goals.

Impact Story Photo Release

The impact story photo release provides an opportunity to share how a patient or canine health research has positively affected grantees, with permission to use your story and photo in CHF communications.

STEP 3

Final Reporting

At the conclusion of your project, submit your final report using the provided templates through the online grantee portal. The final report highlights the impact of your work and its significance in advancing canine health research. Final financials must also be submitted within 45 days of the grant end date to track and document all grant funds according to the approved budget.
